While we may be able to conduct your original consultation thru Telehealth, we may ask that we visit our clinic for further evaluation or treatment.
Before your TeleHealth appointment can take place we will need you to fill out the Appropriate Registration Forms, Provide a Emailed Copy of your ID, Provide a Emailed Copy of your Insurance Card (If Applicable), and if you are self-paying/financing we will need to process your payment before hand.
Once you have Booked your Telehealth Appointment, Fill out the Contact Form on our Website or Call us at 832-831-9433 to collect your payment, insurance information, and so we can send you the appropriate paperwork
